Home Care Packages
Home Care Packages (HCP) support older people to stay living in their own home as independently as possible for as long as possible.A co-ordinated package of home care and services provide people with choice and flexibility in the way that their care and services are delivered to them.
All home care packages are delivered using a Consumer Directed Care (CDC) approach. CDC supports the consumer in the decision making process.Northern Settlement Services staff will work with consumers to support them to identify goals that are important and meaningful to them. Staff will then support, or directly assist, consumers to arrange care and services that will support them to meet their goals.Consumers have the choice of self-managing aspects of the package of services or these services can be coordinated by a Case Manager who is a member of a dedicated group of professionals who understand the cultural and language requirements of people of Culturally and Linguistically Diverse (CALD) background.Greater transparency is also available via monthly statements about how packages are funded and how those funds are spent.It’s all about what is important to individual people and what will fulfil them to lead a great life!The following care and services can be provided by Northern Settlement Services Home Care Packages:► Help around the house – cleaning and housework, personal laundry, home maintenance and modifications, lawn and garden care, spring cleaning► Looking after you – assistance with showering and bathing, dressing and grooming, toileting and personal hygiene, continence management, foot and nail care, medication monitoring and assistance, nursing care including wound, diabetes and pain management► Health and Nourishment – visiting your doctor or other health professionals, encouraging and accessing healthy food choices, grocery shopping, meal planning and assistance with meal preparation► Lifestyle Enrichment – assistance with shopping, banking, paying bills, exercise groups, individual or group social participation and remaining connected with your local community.What will a Home Care Package cost?Home Care Packages are subsidised by the Commonwealth Government. Consumers may be asked to pay:► A basic daily fee of up to 17.5% of the single basic Age Pension► An income-tested care fee if your income if over a certain amount.Allowances and subsidies are available for those families who are financially or socially disadvantaged.
